Rinse the red lentils and place them in a pot. Add the water and cook over medium heat for about 20 minutes, until the lentils are completely tender and have absorbed all the water.
Once the lentils are cooked, remove from heat and add the fine bulgur. Cover the pot and let it rest for 15 minutes, until the bulgur has absorbed the water and swollen.
Heat the olive oil in a separate skillet. Add the finely chopped onion and sauté until softened. Add the tomato paste and sauté for 1-2 more minutes. Add the cumin, crushed red pepper flakes, black pepper, and salt, and stir to combine.
Transfer the swollen bulgur and lentil mixture to a wide bowl. Add the sautéed onion and tomato paste mixture, lemon juice, chopped parsley, and green onions. Knead thoroughly until all the ingredients are evenly combined.
Once the mixture has cooled slightly, lightly wet your hands and take walnut-sized portions, squeezing them in your palm to form small oval patty shapes.
Serve the red lentil patties on lettuce leaves.
